Responsibilities


Core Responsibilites and Essential Functions
  • DIRECT PATIENT CAREn Assess and assist with managing patient's total needs following physician orders with RN oversight and utilization of nursing processn Performs routine nursing procedures to meet the need of the patient under the supervision of RNn Meets established productivity standardsn

  • COORDINATION OF SERVICES FOR EPISODE OF CAREn Coordinates care and services with RN and other team membersn Informs RN, therapist, and/or clinical supervisor of changes in patient's condition, as appropriaten Collaborates with RN to develop and revise individual plan of care based on patient need and physician ordern Perform other duties as assigned by Home Health leadership

  • PATIENT / CAREGIVER EDUCATIONn Serves as patient educator to foster self care or caregiver support for the home patientn Be aware and report concerns to authorities or agency leadership on any issues that affect patient or staff safetyn

  • STAFF EDUCATION AND PROFESSIONAL DEVELOPMENTn Participates in the orientation and education of new employees and nursing students as appropriaten Responsible for attending continuing education and in-services as directed and neededn Accepts responsibility to remain current in clinical knowledge for the care of the home health patientn

  • COMMUNICATION AND DOCUMENTATIONn Responsible for accurate completion of required documentation within established timeframe.n Demonstrates knowledge and appropriate application of regulatory guidelinesn Participates in care team conferences with co-workersn Responsible for checking voice mail box and secure messaging a minimum of once daily.n

Qualifications


Required Minimum Education
  • Graduate of an accredited School of Practical Nursing Current State of Georgia professional licensure. n Required

  • Required Minimum Experience
  • minimum 1 year medical/surgical nursing practice in an acute care environment Preferred

  • Required Minimum Skills
  • Positive leadership, problem solving skills, strong organizational and time management skills.

  • n
  • Strong assessment and clinical skills.

  • n
  • Strong written and verbal communication skills.

  • n
  • Ability to write and follow directions to patient's homes.

  • n
  • Use of Windows based computer, utilizing home health patient software, email, internet, computer based learning modules, etc.

  • Required Minimum License(s) and Certification(s)
  • Lic Practical Nurse 1.00 Required 1.00

  • n
  • Basic Life Support 2.00 Required 2.00

  • n
  • Georgia Driver&s;s License 3.00 Preferred 3.00

  • n
  • Private Vehicle insurance 4.00 Preferred 4.00

  • Additional Licenses and Certifications
    Salary:
    $26 per hour

Estimated Salary: $20 to $28 per hour based on qualifications.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
